What form is needed to change the name of a dealership with DMV?

To change the business name with DMV and for it to be reflected on your dealer license, you must complete the OL-21 Application for Modifications to an Occupational License form and notify your DMV Inspector.

You will also need to notify BPA of the changes as well.

You will need to complete the Reg 4026, submit the corporate meeting minutes, submit a copy of the new OL license, and submit a letter on company letterhead explaining the name change.
