When is a DMV 14 Change of Address form used?
It is used to clarify the mailing address/residence on registration and titling documents. When an owner’s residence and mailing address are in different cities, the owner’s mailing address should be keyed on the registration documents.
- Both addresses should be shown on the application. A DMV 14 should be completed showing both the residence and mailing address and should be submitted with the application.
- Notify the owner that only the mailing address will be shown on the registration, but the residence address will subsequently be updated on the vehicle record.
