Guidelines for Correcting a Date on DMV Documents
If you find an error in the date on your DMV documents, it is important to correct it properly. Below are the steps you should follow:
Strike Through the Incorrect Date
You may simply line through the incorrect date on the document. Make sure this strike-through is clear and legible.
Add Your Initials
After lining through the incorrect date, add your initials next to it. This signifies that you have made an intentional correction.
Attach a Statement of Facts
Prepare and attach a "Statement of Facts" detailing why the date needs correction. Include information such as:
- The original (incorrect) date.
- The corrected (correct) date.
- An explanation for why this change is needed.
