You may simply line through the incorrect date on the document. Make sure this strike-through is clear and legible.

Add Your Initials

After lining through the incorrect date, add your initials next to it. This signifies that you have made an intentional correction.

Attach a Statement of Facts

Prepare and attach a "Statement of Facts" detailing why the date needs correction. Include information such as:

  • The original (incorrect) date.
  • The corrected (correct) date.
  • An explanation for why this change is needed.
