Creating a Work Order Request

To create a work order request, follow these steps:

Accessing the Logbook

1. Access the logbook and locate the record you need.

Printing the Work Order Request Form

2. Click on the three lines on the far right of the entry to find options.
3. Select "Print Work Order Request Form."

Filling out the Form

4. In the form, enter a comment indicating your purpose, such as “complete transfer and correct to 'Actual Mileage.'”
5. If you're ready to clear an item, change any necessary settings accordingly.
6. When adding descriptions in the work order printout, select "clear" instead of "post fees."

Completing and Submitting

7. Sign the completed work order and prepare all relevant documents for submission.
8. Mail all original documents to the same address used for your normal bundle, using the work order as a coversheet.

Note:

If you are submitting for post fees instead of clearing an item:

  • You can email SPU with copies of your documents along with your signed work order.

Make sure not to email anything if you’re clearing an item; just mail in originals only.
